Life cycle of In Vitro Fertilization (IVF)

In Vitro Fertilization (IVF)

IVF treatment lifecycle

This is a treatment for fertility where an egg is fertilized using a sperm on the outside of the womb of a woman, the lifecycle of IVF treatment is carried out in different stages:

Egg supply boost in women

The first thing that is done is to actually prescribe some drugs that control egg production. The drugs are meant for egg supply boost in women. In this case, eggs ofa larger number can actually be fertilized thus the clinic is able to select from a wide range of already fertilized eggs. The medication given for purposes of fertility replaces or improves the hormones that occur naturally.

Progress check

IVF2-300x225A clinic carries out scans of vaginal ultrasounds that are able to monitor egg development. Blood tests are also done so as to be able to be in step with the levels of estrogen as they rise and their production. In this way, help is rendered in maturity of eggs monitoring. Some 38 or 34 hours prior to egg collection, a hormone injection is give as a way of helping the eggs to mature fully.

The collection of eggs from a woman

During IVF, retrieving eggs from a woman is done by sue of minor surgery. This is done within a room which is sterile and an anesthesiologist present. Mild anesthesia is usually used and the procedure takes an hour or so. Before the procedure, one is advised not to take any food since the night before and also avoid taking chewing gum, sugar candy, water, drink or any form of food. Using a vaginal ultra sound, location of ovarian follicles is done. Once successful, a needle is used through the vagina’s back wall and directed to the follicles and the collection of eggs is done.

Once the process of egg collection is done, one can be able to take food but more preferably light food and fluids. You should not work or even drive on that day and it is recommended that a person actually takes a rest in their home.

Collection of sperm from a man

When the egg is being collected, a sample of sperm is also given. The laboratory in turn takes the sperm and places it in an incubator for a while. A process called sperm clean is carried out so as to be able to actually set aside the sperm which seems to be the healthiest. When sperm from a donor is used, this process is still applied.

During the lifecycle of IVF treatment process, the interval that is in between sperm collection and last intercourse shouldn’t exceed 3 days. If this is the case, semen has to be renewed before prolonging the period of abstinence.

Embryo development and process of fertilization

IVF3After the egg is collected, it is taken to a laboratory. To initiate this process, a petri dish is used where eggs will be placed. The petri dish has a solution that has egg nutrients that are specialized. They sit in the petri dish for at least two or three hours. The sperm is then incubated with eggs for at least 16 to 20 hours. After this, they are checked so as to tell whether fertilization has actually taken place. When fertilized, the eggs are referred to as embryos and are taken to a laboratory which is special. They stay there for 24-48hrs. After this duration, the conditions are re-checked.

IVF4As an alternative, patients that have problems related to male infertility have a choice where they can get an ICSI. This is a process where a single sperm will be injected to the egg directly. This process is usually carried out by use of a microscope. The devices used are micromanipulation such as micropipettes, micro injectors as well as micromanipulators. A pipette for holding actually stabilizes the oocyte when mature. A needle which is hollow is then passed into oocyte middle part through the oolemma. A single sperm is then released.

Preparation of the endometrium

A prescription of progesterone and estrogens is given so as to have the endometrium prepare.

Transfer of the embryo

This involves a very simple procedure and it requires no anesthesia. After the eggs have been fertilized up to two or even five days, a morphological criterion is used to select the ones that are healthiest. These are the one placed in the womb. A catheter is used to guide the embryo into the uterus. The embryos can be up to four but this highly depends on the medical history as well as age of the recipient. One is then advised to take a rest between 24 to 48 hours. Any other embryos that were successfully fertilized are then frozen and can be used at a future date.

Your partner can be [present during this procedure and one need to be driven home on completion.

Blood spotting is common after the process but it is very mild. A pregnancy test is then done even after spotting.

Pregnancy testing

After 14days of the transfer of the embryo, testing is done. If such a test is on a weekend, it will have to be postponed or done on a Friday if it falls on Saturday or Monday in case of Sunday. After the test, you will be able to know whether or not you are pregnant. Sometimes you may get a result that is weakly positive. This can be caused by:

  • Late normal embryo implantation.
  • Discontinued pregnancy
  • Ectopic pregnancy
  • A Lab error

HCG monitoring is very important. Whenyou do the first test, you should wait two days so as to have a repeat of the same. The blood work is aimed at determining whether the pregnancy is going well. The level of HCG doubles in three or two days.

After the retrieval and four or three weeks have passed, ultrasound will be performed. This ultrasound is quite important as it evaluates the possibility of multiple pregnancies, ectopic pregnancy or miscarriage. An ectopic pregnancy can occur in 2-4% of the IVF pregnancies. An early diagnosis can actually be treated using medication. If your gynecologist determines that a fetal heartbeat is present, one is referred to the obstetrician.

If the pregnancy tests turn out to be negative, one should get in touch with us so that you may give medications and other instructions. If treatment is stopped, your period should come in three or even five days’ time if at all bleeding hasn’t started. The period may not be what you normally experience as it may be longer or shorter or it may be heavier or lighter. If one doesn’t get their period within a week, get in touch with us so that the blood test can be repeated.

When one invests money and time so as to get the ART, a negative test can be shocking. Talk to your physician so as to know the best way forward.
